Abstract

Penelitian ini mengemukakan dua cars terapi pada karsinoma hepatoselular lanjut seria membandingkan hasil yang dicapai pada 20 pasien. Berdasarkan respon tumor masing-masing setelah terapi, temyata kemoterapi dengan infus berkelanjutan menunjukkan hasil yang lebih balk dan lebih efektif. Akan tetapi cars pengobatan itu juga menyebabkan efek samping yang lebih berat.Kedua cars terapi di alas memberikan angka ketahanan hidup (survival rate) 70% untuk 6 bulan dan 40% untuk 1 tahun. Sebagian besar pasien dalam penelitian ini mengalami gangguan fungsi hepar yang berat dan stadium penyakit sangat berperan terhadap hasil terapi.Key Words : hepatocellular carcinoma transcatheter arterial chemoembolizati on infusion chemotherapy anticancer drugs angiography

Abstract

During three years periode from August 1989 to August 1992 eight patients underwent anterior resection for rectal cancer, five patients in Sardjito Central General Hospital and three patients in Patmasuri Special Surgery Hospital. Five patients with high rectal cancer and three patients with middle rectal cancer underwent anterior and low anterior resection respectively. Intestinal obstruction due to local recurrence occured four months after operation in oae patient who underwent low anterior resection and cancer regression after preoperative radiation occured pathologically in one patient.Key W ords: high rectal cancer anterior resection middle rectal cancer low anterior resection cancer regression

Abstract

Left bundle branch block is very rare in children. A boy, 5 years old with diphtheritic myocarditis had left bundle branch block in ECG pattern during the first days in hospital. He had received basic DPT vaccination 4 1/2 years ago, when his age was three months.The prognosis of left bundle branch block in diphtheritic myocarditis is usually poor. However, as he received DPT vaccination in the last 5 years the prognosis was tending to good. After 11 days hospitalisation, the ECG pattern suddenly reversed to normal. The follow up- through outpatient department has no further complications, despite he discharged with still high LDH serum leveLThe purpose of this study is to report a very rare case, and this case hopefully to be the last diphtheritic disease in Yogyakarta, due to a very good coverage of vaccination.Key Words: diphtheria diphtheritic myocarditis left bundle branch block DPT vaccination LDH enzyme

Abstract

Since the first description in 1925, thalassemia has been studied intensively and extensively. Thalassemia is inherited by Mendelian recessive genes, in which there is a mutation or deletion of the DNA nucleotide or results in defective production of one or more globin chains of the hemoglobin. There are two important forms of tha/assemias, a- and ft-thalassemia that are health problems in the tropical belt, extending from Africa and Meditenanian countries up to West Melanesia. The molecular studies of thalassemia and other hemoglobinopathies had discovered such a lot of abnormal hemoglobin structures and pathogenesis of the disease, so that the hemoglobin is now indeed a paradigm for our understanding of gene action at the molecular level. The thalassemia occurs essentially due to deletion, whereas 13thalassemia is mostly due to mutation of the gene(s). The understanding of the molecular pathogenesis of thalassemia is very essential for the diagnosis of the trait and especially for prenatal diagnosis, and in the future might be for genetic therapy.Key Words: thalassemia gene deletion gene mutation polymerise chain reaction haemoglobin

Abstract

The paper presented a case of hypothyroidism in patient with Graves disease after have long been treated with antithyroid agent. The cause of hypothyroidism in this case might be (1) the effect of anti-thyroid agent, (2) natural history of Graves disease as a result of autoimmune mechanism, and (3) coincidental presentation of Graves disease and condition causes hypothyroidism (hashitoxicosis).A 39 year old woman, household , admitted to Sardjito Hospital because of general fatigue and facial edema. She was previously treated as Graves disease with PTU (propylthiouracil) 100 mg 3 times 100 mg/day for 2 years. Because of allergic reaction against PTU, the drug was replaced by Neomercazol®. One year later the patient complaining body weight (BW) gain, with the laboratory investigation showed T3 = 0.78 µg/d1 (N: 0.6-1.9); T4 = 2436 µg/d1 (N: 45-100). Six months before admission she has not taken any drugs. On admission, BW 60 kg, other vital signs were in normal limits, palpebral edema, T3 = 0.3 itg/d1; Ta = 2.0 jig/d1; Total cholesterol 329 mg/dl; LDL-cholesterol 238 mg/dl; triglycerides 298 mg/d1; Anti-thyroglobulin 1/1280 (N: negative); anti-microsomal negative; other laboratory results revealed normal values. She was diagnosed as hypothyroidism post treatment of Graves disease. She was treated with Thyrax® 1 tablet daily, and one year later she was in a normal condition.All patients with Graves disease should be followed up regularly eventhough the clinical condition already subsided, since other autoimmune disease may coincidentally present.Key Words: hypothyroidism Graves disease hashitoxicosis antithyroid agents autoimmune disease
